Find the Network That Works for You

ACA plans come in four network types, each offering different flexibility and cost structures.

HMO

Health Maintenance Org.

Lower premiums with a primary care physician who coordinates your care. Referrals required for specialists. Must use in-network providers.

EPO

Exclusive Provider Org.

No referrals needed but must use the plan's network of providers. No out-of-network coverage except emergencies. Lower premiums.

PPO

Preferred Provider Org.

Most flexibility — see any doctor in or out of network without a referral. Higher premiums but greater choice of providers.

POS

Point of Service

Hybrid plan combining HMO and PPO features. Requires a primary care doctor but allows out-of-network visits with referral.

60% insurer pays

You pay ~40%

Lowest monthly premium. Highest out-of-pocket costs when you need care. Best if you're generally healthy and want emergency protection.

Silver
70% insurer pays

You pay ~30%

Moderate premium and costs. Eligible for Cost Sharing Reductions (CSR) based on income. Most popular choice for families.

Gold
80% insurer pays

You pay ~20%

Higher premium but lower out-of-pocket when you need care. Good if you use medical services frequently or have ongoing prescriptions.

Platinum
90% insurer pays

You pay ~10%

Highest premium, lowest out-of-pocket expenses. Best for those who need regular, substantial medical care throughout the year.

Who Qualifies for ACA Coverage?

To enroll in an ACA Marketplace plan, applicants generally need to meet these requirements.

Legal Immigration Status

Must reside legally in the United States. Valid immigration documents and status are required to enroll in a Marketplace plan.

Social Security Number (SSN)

A valid SSN is required to verify identity and process enrollment. ITIN is not accepted for ACA Marketplace plans.

Filed Federal Tax Returns

Household income reported on tax returns is used to calculate eligibility for premium tax credits and subsidies.

Not Enrolled in Government Programs

Applicants currently receiving Medicare, Medicaid, or CHIP are generally not eligible for ACA Marketplace subsidies.
